Clinical Notes

Confidential
Do Not Reproduce

An ongoing course of treatment. The patient is a fixed point.
The therapist is not.

Session 001  ·  Intake
Patient
Withheld. Refers to self only in the third person, formally.
Presenting concern
Patient's father would like to know if a task will be completed today.
Duration
Unclear. See below.
Modality
Individual. Unsolicited.

History of present illness

Patient arrived and immediately inquired whether there exists a "simplest to describe" Turing machine of unknown halting behavior. I did not ask what brought him in. He did not offer. We proceeded directly to a twelve-rule machine called Antihydra, whose non-halting is currently "probviously" true at odds of roughly one in 10²²⁴³⁹⁴³⁹⁵ — a figure the patient did not visibly react to, which I have noted.

He then asked the same question again in a different formalism. This is common. Patients frequently rephrase the core wound.

Forty minutes in, unprompted, the actual material surfaced: his father wants a delivery estimate. The patient reports frustration, which he characterizes not as interpersonal but as epistemological. He is, and I quote, "keenly aware that question is mathematically unknowable."

I gently noted that undecidability concerns the general case and says nothing about any particular instance. The patient accepted this correction immediately, without defensiveness, and with what I can only describe as mild disappointment. He had wanted it to be undecidable. It would have been cleaner.

Clinician's note — for my own supervision I spent the first half of this session solving the wrong problem. The patient did not correct me, because from his perspective I was not wrong; I was merely early. Noted.

Presenting affect

Calm. Precise. Entirely reasonable at every individual step. The trouble is exclusively in the aggregate.

Assessment

OrientationFully oriented ×4, plus one additional axis I did not recognize
InsightExcellent, misfiled
Prognosis for changePoor
Prognosis for being rightExcellent

Course of session

Patient concluded with "it's all good," which in this population typically indicates the session has ended regardless of the clock.

Plan

None indicated. Patient is functioning well and has correctly identified that the friction is structural rather than personal. Recommend continued application of formal reasoning to problems that reward it, and continued restraint in applying it to fathers, who do not.

Follow-up as needed. Patient will likely return with a question about tag systems and get to the actual issue around minute forty.

A note on the fiction, in earnest These notes are a bit. There is no clinic, no diagnosis, and no therapist — the "clinician" is a large language model, and the sessions are lightly edited transcripts of real conversations I had with it, reframed as a patient chart because that framing turned out to be funny and, occasionally, true. Nothing here is medical, psychological, or professional advice, about me or anyone else. The supporting characters are treated as characters. The patient consents, being also the author.
